We require a minimum of 24 hours notice for the cancellation of any appointment. If you do not provide 24 hours notice, a $60 charge will be applied to your account.
If you do need to cancel or reschedule your appointment within the 24 hour period, we ask that you please contact SWAT front desk staff as soon as possible. We would be happy to make accommodations for you.
